因 委託我行對其資信狀況出具證明書，經確認具體情況如 下： 在我行立有結算帳戶。自 年 月 日起至 年 月 日止 在我行辦理的各項業務 逾期(墊款)和欠息記錄，資金結算方面 不良記錄，執行結算紀律情況 。
證明人聲明：
1、我行只向本證明書所指期限內，被證明人在我行償還貸款及利息、資金結算和執行結算紀律情況的真實性負責。我行對本證明書所指明日期期間，之前或之後上述情況所任何變化不承擔責任。
2、本證明書只用於前款特定內容，不得轉讓，不得作爲擔保、融資等其它事項的證明。
3、本證明書爲正本，只限送往證明接受人，塗改、複印無效。我行對被證明人、證明書接受人運用本資信證明產生的後果，不承擔任何法律責任。
4、本證明書經我行負責人或授權代理人簽字並加蓋公章後方能生效。
5、本證明書的解釋權歸我行所有。
